Lost/Stolen Devices:

Staff/Faculty – In the event that your CHSU issued laptop, personal mobile device, access badge or photo ID becomes lost or stolen: 

1.If this occurred while on campus, ask the front desk receptionist if someone turned it in or email Facilities at facilities@chsu.edu for assistance with locating them. If your laptop was stolen off campus, file a police report and notify your supervisor.
2.Notify IT Services so that they can disconnect CHSU email from the stolen device, request a new badge and the de-activate the lost badge.
3.IT Services will issue you a loaner laptop and initiate your replacement laptop. As well as assist with changing your CHSU password.
4.Email a completed Project Request form (located in N:\Forms) to the marketing department for the replacement of your ID photo card.
