New Medication Management Patient Care Program Offered at Clovis Pharmacy School

California Health Sciences University (CHSU) is proud to announce the launch of the CHS Clinic, a new medication management patient care program located on the CHSU College of Pharmacy Clovis campus.

The CHS Clinic (CHSC) is a pilot program offering the patients of partnering physicians or providers specialized health and medication management services, especially for those patients with chronic diseases. Numerous studies have already documented multiple benefits and positive outcomes when pharmacists provide medication management services in integrated care teams. The CHS Clinic program aims to reestablish the physician-patient-pharmacist relationship as an on-going initiative.
As part of the university’s mission to improve the healthcare outcomes of people living in the Valley, this program was formed working in close collaboration with the Community Care Health Plan and well-respected, local physician Ali Fayed, MD, who practices Internal Medicine with the Community Medical Providers medical group.

One Year Pilot Program

During the one year pilot program, the clinical pharmacist faculty working through the CHS Clinic program will provide collaborative patient care and medication therapy management to qualifying patients of the Community Care Health Plan, which will include employees and dependents from both Community Medical Centers and Santé IPA that have selected an HMO option.
“We expect our collaborative arrangement with the CHS Clinic program to greatly benefit our patients as they receive the specialized care necessary to manage their chronic diseases and the physician remains the central member of the patient’s health care team and receives any new and evolving information about our patients,” stated Ali Fayed, MD.
Aldo De La Torre, CEO of Community Care Health Plan, agrees. “We are confident that this added benefit will provide our physicians with an additional resource to assist in the care of their patients and, ultimately, improve the health of our members.”

Specialized Patient Care

Patients treated at the CHS Clinic will have the opportunity to optimize their treatment of diabetes, cardiovascular disease, dyslipidemia, hypertension, and other chronic diseases and to improve patient understanding and management of their health.
“The clinical pharmacist faculty at the CHS Clinic will provide patient-centered care with active involvement of patients and their families in decision-making about individual options for treatment and self-management,” stated Dr. Wendy Duncan, SVP for Academic Affairs, Provost and Interim Dean of Pharmacy at CHSU. “The idea is that we sit side-by-side with the patient and his/her family and caregivers to co-design their plan.”

Student Learning Opportunities

CHSU College of Pharmacy students will have the opportunity to rotate through the clinic under direct oversight of clinical pharmacist faculty during their service learning experiences.
“The CHS Clinic program at CHSU College of Pharmacy offers a unique opportunity for students who will be trained in conducting patient-centric care under the supervision of our clinical faculty – to help patients manage their chronic diseases in collaboration with physicians and other members of their healthcare team,” explained Dr. Duncan. “We hope our graduating pharmacists will continue providing compassionate, individualized care to empower patients in the Valley to help reduce the health care professional shortage and fulfill our mission.”
Students from other health care disciplines at partnering universities will also have interprofessional practice experiences at the CHS Clinic.
“Our hope is that the CHS Clinic program helps our partnering physicians to better serve their patients by making health care more accessible, improve patient care outcomes, reduce repeat hospital and emergency department admittance and reduce health care costs,” said Dr. Duncan.
